Children must turn three by December 31st. All children must be fully toilet trained. Toilet trained means they are able to use the toilet by themselves.

Before entering school, all children in the state of Hawaii must turn  in Health Form 14 and the Pre-K Health Supplemental Form.  They must be  completed and signed by the child’s licensed medical physician and  submitted along with a copy of the child’s birth certificate. Health  Form 14 & the Pre-K Health Supplemental Form must include updated  records of the child’s physical exam, immunizations, TB clearance, any  allergies and/or medical restrictions with doctor’s note.

The Registration Form and Registration Fee must be submitted during registration.

Submission of the Registration Form and Registration Fee will guarantee enrollment at Pali View Baptist Preschool. Please pay by personal check,  cashiers check or money order. No cash or credit cards are accepted.

The Registration Fee is non-refundable, non-transferable and not applicable toward tuition.

The Enrollment Packet will be emailed or mailed out following registration.  All  forms in the Enrollment Packet must be signed, completed and submitted  to the Preschool Office:

– Emergency Information Sheet
– Child Enrollment Information Sheet
– Family, Health, & Social History Form
– Biting Policy Agreement

– Photo Release
– Sunscreen Release
– USDA Enrollment Form
